Full Job Description

Join our Community Supportive and Palliative Care Team as an Assistant Practitioner (Band 4, 30 hours / 0.8 WTE). An exciting opportunity has arisen for an Assistant Practitioner to work within the well-established Community Supportive and Palliative Team. The successful candidate will work flexibly over 4 days Monday Friday 8.30 16.30 (excluding bank holidays).

The service is based at Radford Health Centre and provides specialist support to palliative patients and their families across a variety of settings across Nottingham City including the patients own home.

The successful candidate will work within the team to support both the Nursing and Rehabilitation staff already in post. The role will help to support and improve the delivery and provision of palliative end of life care in the community for all palliative care patients with a life limiting diagnosis, in order to improve quality of life, promote independence and improve the patients and carers experiences at the end of life.

This is a rewarding opportunity for someone with a Foundation Degree, NVQ Level 3 (or equivalent), or demonstrable relevant experience in health, social care or voluntary care. You should have experience of working within a multi-disciplinary team, managing your own workload, maintaining accurate records, and supporting people with palliative or long-term conditions.

You will need strong communication and interpersonal skills, a compassionate and person-centred approach, and the ability to assess needs, support independence, contribute to care planning, and work closely with nurses, therapists and other professionals. You should also be confident working flexibly, carrying out delegated clinical and rehabilitation tasks, and supporting safe, high-quality care delivery.

A driving licence and access to a vehicle for work purposes is essential.
