An exciting opportunity has arisen for an Assistant Practitioner to join the Mental Health Accommodation-Based Review Team. You will be part of a small and supportive team, working alongside three Social Workers, three Assistant Practitioners and a Team Manager. Taking a Recovery and Trauma Informed approach and working in partnership with the mental health service's main service provider, Norfolk Integrated Housing and Community Support Service (NIHCSS), your work will focus on reviewing the needs of individuals receiving packages of support which include a NIHCSS Outreach Service. Your aim will be to ensure the service is effective in meeting the evolving needs and outcomes of those receiving it and to promote effective use of the NIHCSS resource. Your reviews will promote independence and ensure clear identification of SMART outcomes. Geographically, the work will predominantly be in the Norwich and South Norfolk areas. You will also work alongside our key partner, Norfolk & Suffolk NHS Foundation Trust to provide an integrated approach to assessment and support for adults with mental health needs, with an emphasis on s.117 aftercare planning and review.

You will be passionate about making a difference and ready to embrace change. A team player, you will need to demonstrate an ability to work under pressure, be well organised and able to effectively prioritise. Ideally, you will have experience of working with adults with complex needs and have either experience/interest in working within the mental health field of social care. You will have experience, knowledge and commitment to strength-based assessment. The ability to travel is essential. To build meaningful relationships with the people we support, this role requires regular in-person work in both the office and community.
