You will work in a team with other multi-disciplinary professionals including Nurses, Physiotherapists, Occupational Therapists, Administrators and other support staff, on a rota system between the hours of 8am-8pm over a 7 day working period., You will work as a member of the team to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients in our service.
You will:
+ Provide assessment and treatment to patients on the caseload with the guidance of the wider team
+ Maintain a high professional standard of care in adherence to all protocols/policies & guidelines
+ Manage a patients care plan within your scope of practice and with the support of the wider team
+ Provide clinical assistance to other members of the clinical team
+ Maintain accurate electronic clinical records
You will work within our service that is delivered from 8am-8pm, 7 days a week service.
You will be required to respond to referrals across urgent and planned pathways including hospital discharge facilitation.
You will work as part of the team to proactively monitor and manage patients on the caseload to ensure the best outcomes for them.

You will need excellent communication and clinical skills and have experience of working within a team. You will be required to use electronic patient record systems, in addition to this you will have good time management and be able to prioritise your own work load.
You will have many opportunities to develop your clinical skills as well as physical assessment and clinical reasoning with the support of the wider team.
A full driving licence and access to a vehicle is required for this role and whilst we are looking for full time hours, we can consider part time hours.
