Assessment and Enablement Worker

You will be responsible for managing a caseload of individuals; assessing social care needs; developing, co-ordinating and reviewing care support, as well as ensuring that we meet our duty to safeguard vulnerable individuals. You will be committed to working with customers in a person centred way to support them to achieve effective outcomes. You will be supporting people within the community to be able to remain as independent as possible within their own home and reduce the risk of crisis support. Tasks may include but not limited to assessment and provision of low level mobility aids, assisting with home management tasks, provision of items to meet basic needs such as emergency food parcels, supporting individuals with housing, finances, medication and follow up appointments., Our values define who we are and how we operate, by forming the foundation for how we interact with our customers, colleagues and provide our services.
