Assertive Outreach Team Support Worker

As a Support Time and Recovery Worker, you will play a vital role in helping people with mental health difficulties take positive steps towards recovery, independence, and wellbeing. Working as part of our Community Mental Health Service, you will build supportive, therapeutic relationships with people in their own homes and local communities, helping them achieve goals that matter to them. You will also support individuals during their inpatient stay and help them prepare for a successful return home, providing continuity and reassurance at key points in their recovery journey. • Support individuals with mental health difficulties to work towards their personal recovery goals and increase their independence.
• Deliver evidence-based interventions and practical support under the supervision of qualified professionals.
• Help people engage with their care plans, develop coping strategies, and build confidence in everyday life.
• Work closely with carers, families and partner organisations to ensure people receive coordinated support.
• Recognise early signs that someone's mental health may be deteriorating and share concerns promptly with the wider team.
• Promote wellbeing, independence, social inclusion, and access to support around housing, benefits, advocacy and other community resources.
• Contribute to service improvement, quality initiatives and the ongoing development of the team., You will join a Community Mental Health Service that supports people experiencing severe and enduring mental health difficulties living within the community. The service is committed to delivering high-quality, recovery-focused care and works collaboratively across health, social care and voluntary sector organisations to provide the best possible outcomes for local people.
As a Support Time and Recovery Worker, you will manage a varied caseload and provide regular, consistent support to individuals. You'll work alongside experienced mental health professionals and fellow Support Time and Recovery Workers, ensuring patients receive the right support at the right time. This role offers the opportunity to develop valuable skills in community mental health, recovery-focused practice and evidence-based interventions. You'll receive regular supervision, support and opportunities to contribute to service development, quality improvement and team initiatives. This role may include working a flexible shift pattern, including evenings, weekends and nights where required by the service. • Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance in order to travel throughout the Trust, to meet the needs of the service.
  
  NVQ Health and Social Care or equivalent, or willing to undertake full training to achieve this standard, Experience of working with people e.g in a caring situation, dealing with the general public.
• Experience of working in community settings.
• Experience of the assessment process and positive risk management., Previous experience in caring for people with a severe mental illness.
