Full Job Description

  • The post holder will work as a Band 6 Lead Nurse within the team you will be responsible for the coordination of care & support for an identified caseload of service users.
  • Maintain NMC registration / re-validation and adhere to professional guidelines and code of conduct.
  • Work within the framework of relevant policy and legislation both local and national.
  • Provide supervision to junior members of the staff team.
  • Will be responsible for the planning implementation and evaluation of nursing care within agreed parameters and have a working knowledge of local and national policies & legislation.
  • As an active member of the team undertaking the assessment & screening of patients referred to the AOT service.
  • Will be responsible for ensuring effective communication pathways exist with both internal and external services.
  • Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.
  • Will be responsible for identifying and contributing to the training need, of themselves and those who they supervise, necessary in providing quality evidence-based care as appropriate.
  • Support colleagues in the effective assessment and the delivery of interventions to individuals within their assigned caseload.

    This is a 12 months fixed term position secured through Better Care Funding, which is now in its second year within AOT. This is an exciting opportunity for the right candidate to positively engage with and change the lives of service users who have a severe and enduring mental illness and who have disengaged with mental health services. Currently the team is funded to provide a 5 day a week service, but it is possible that the strategic goal will develop to move to a 7-day service, dependent on additional funding. T The AOT offers a city-wide service and the aim of the team is to work with physical health, social, and economic services to reduce exclusion and to promote the psychological needs of service users where due to the lack of visibility access to services are often denied to them. You will need to have the enthusiasm for, and experience of, working with service users with complex needs & who experience difficulty in engaging with mainstream services. You will be responsible for setting supervision and support systems to junior AOT staff in their delivery of interventions, and to deliver excellent evidence-based care to patients within the service.
