We are excited to offer a fantastic opportunity to join our South Adult Social Care Occupational Therapy Team as an Occupational Therapy Assistant., In joining our team, you will be influential in supporting community services with a strong focus on rehabilitation, education and prevention, and self-management / lifestyle advice for long term conditions. Our aim is to maximise function, prevent deterioration and ensure both safe discharge and / or follow up in the community with short- and longer-term rehabilitation programmes ensuring adaptation to life changes.

We are looking for enthusiastic Occupational Therapy Assistant with:

o Keen interest & / or experience of working in rehabilitation services and can demonstrate a strong understanding and vision of specialist therapy and service needs.o To assess new service user referrals and care/support needs.

Care/support plans may include:o Promoting independence/self-management and maintaining safetyo Increased functional abilityo Improvement in health & wellbeing including mental healtho Prevention and reducing of health inequalitieso Admission avoidanceo Case management/ treatment/ care planso Proactive in developing sound clinical skills in the assessment and management of patients in the rehabilitation and / or community environment.o Deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques where appropriate.o To take accountability for allocated assessments, planning and implementation of care packages/ programmes of support within their caseload.

This opportunity is a permanent fulltime position with ASC team, at Band 4. Applicants would benefit from previous Adult Social Care or Community Occupational Therapy experience.

You will work as part of our team of experienced Occupational Therapists, Assistant Practitioners and Assistants to provide multidisciplinary services to adults with a range of conditions across the different specialities.
