Area Co-ordinator

As an area co-ordinator, you will play a vital role in communicating with and assessing people who want to live independently, confidently and well in their own homes. You will create support plans for them once you've assessed we can meet their needs. You will support and supervise a small team of locally based Living Well Assistants who provide the day-to-day support to clients. The Area Co-ordinator is the first face new clients will see and provides an important link between the office-based team and the Living Well Assistants working in the community supporting older people.
