Full Job Description

The AP will provide clinical expertise and leadership to deliver same-day pre-operative anaesthetic assessments for complex cancer pathway patients. They will be responsible for triaging patients according to medical complexity, the performance of comprehensive preoperative assessments and utilisation of clinical reasoning to formulate a shared management plan before surgery to help deliver the 62-day cancer pathway standard. The AP will run planned pre-operative assessment clinics for complex pre-operative patients listed for major surgery. They will be skilled in risk stratification, able to refer for targeted investigations, optimisation & high-risk multidisciplinary team input within the limits of their capability & subject to minimum supervisory standards (NHSE, Workplace supervision for advanced practice 2025 refresh) & clinical governance (Advanced Practice Governance Maturity Matrix; NHS employers). The AP will respond to colleague queries for anaesthetic notes reviews at a rate agreed locally depending on clinical responsibilities and workload & within the limits of their capability. The AP will be formally involved in national perioperative audit processes, including NCAPC & PQIP data collections.   • Demonstrates a critical understanding of their advanced level of responsibility and autonomy, within the limits of their own capability, to include when working with complexity, risk or uncertainty in undifferentiated patient consultations
  • Facilitates own professional judgment identifying when to escalate and seek senior support/guidance, critically reflecting on practice, self-awareness and emotional intelligence, Works in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking and physical assessments, holistic assessments, identifying risk factors, mental health assessments, requesting/ undertaking and/or interpreting diagnostic tests, and conducting health needs assessments)
  • Utilises effective communication skills through a variety of methods/settings, supporting people in making decisions, planning care or seeking to make positive health changes, promoting person-centred approaches in health and care
  • Delivers effective communication skills with colleagues both internally and/or externally, in verbal, non-verbal and written communication methods, adapting to changing parameters as required; which includes thorough accurate documentation in patient records as required
  • Provides and receives highly sensitive, complex or contentious information relating to patient care, and communicates information to patients, families or carers with respect, compassion and empathy, including breaking 'bad news' or facilitating difficult conversations with respect and compassion
  • Delivers a high degree of proficiency and clinical acumen in relation to decision-making skills, to inform clinical reasoning approaches when dealing with differentiated and undifferentiated presentations and complex situations; synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses, treating, discharging, referring to alternative specialities or escalating cares as required, within agreed scope of practice and with support from the consultant anaesthetic and clinical leadership team.
  • Able to competently perform a range of clinical procedures and lead diagnostic investigations, as required by the service and the scope of practice for the role:
  • Phlebotomy Peak flow/spirometry ECGs
  • Actively endeavours to continually develop, broadening knowledge and skills in advanced clinical practice, in order to initiate, evaluate and modify a range of evidence-based interventions, which may include the prescribing of medications, therapies, lifestyle advice and care, as required by the service and role
  • The AP will be registered as a non-medical prescriber, qualified to prescribe medications safely and effectively in line with their scope of practice, utilising local or organisational guidelines, national guidelines, and adhering to the Royal Pharmaceutical Society (RPS) Prescribing Standards, and those of their professional regulatory body (e.g. NMC, HCPC, GPhC), The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

    Registered Healthcare Professional (NMC, HCPC or GPhC)
  • First level degree (HEI Level 5 or 6) or equivalent, in undergraduate base profession
  • Completed MSc L7 Advanced Practice or equivalent - in relevant Advanced Practice field; including independent prescribing
  • Specialist post-graduate education / qualifications, or training relevant to area of clinical practice for AP role (e.g. IRMER)
  • Registered Independent/Non-medical Prescriber
  • Desirable criteria
  • Mentoring, Facilitating Workplace Learning or Clinical Supervision Qualification
  • Leadership Course or Qualification
  • Life Support Course relevant to speciality (ILS, BLS, ALS)
  • Professional AP Credential - From recognised professional body pathways, or NHSE, Significant experience and expertise within pre-operative assessment for the AP role (at AP or tAP level), following consolidation and application of required advanced practice training
  • Proven evidence of management ability and leadership skills
  • Evidence of ongoing CPD, including multi-source feedback and workplace-based assessments and evidence of capabilities, within own professional portfolio
  • Excellent clinical acumen and practical procedural skills (with evidence of competency and capability) as an independent practitioner, as per Job Description for role
  • Knowledge of clinical governance issues and measures of quality
  • Experience of clinical audit, service evaluation or improvements in practice
  • Ability to appraise evidence, using analytical and problem-solving skills to inform decision-making, and ability to respond to sudden unexpected demands

  • Demonstrable evidence of research capability, or the application of research evidence to own clinical practice
  • Existing technical clinical procedural or interpretation skills relevant to clinical service (e.g. overnight oximetry monitoring)

  • Commitment to evidencing own professional development, capabilities and competency development via work-based assessments and use of a portfolio

  • Ability to recognise and analyse complex situations, utilising decision-making skills to take appropriate action
  • Evidence of teaching and sharing knowledge with others in the MDT or externally to organisation
  • Demonstration of leadership in clinical practice
