Full Job Description
We're looking for an experienced clinical pharmacist to join our Acute &Emergency Medicine team at the Royal Stoke site. You'll help deliver a high-quality clinical pharmacy service to high-acuity and complex patients across our Acute Medical Unit, AMRA, AEC, SSU and to the Emergency Department.
You'll work closely with pharmacy colleagues and multidisciplinary team members, with opportunities for education, development, research and networking. You will also train and support the development of junior pharmacists within the Acute Medicine rotation., Deliver a high quality, patient focused clinical pharmacy service to Acute/Emergency Medicine
Prioritise and review high-risk patients (e.g. those with epilepsy, diabetes, Parkinson's disease), ensuring their medicines are prescribed and administered within a timely manner
Utilise your independent prescribing skills daily, working within your scope of practice
Provide prescribing support for complex medicines e.g. aminophylline, vancomycin
Support patient flow with mechanisms for obtaining discharge medicines both in and out of hours
Review adverse incidents and take appropriate action to reduce future risk
Review and develop guidelines and PGDs.
Line manage and train and develop junior pharmacists and trainee pharmacists
Work with the Advanced Lead Pharmacist for Acute and Emergency Medicine on the strategic development of the clinical pharmacy service to the Emergency Department and Acute Medicine.
Has experience of delivering evidence-based, cost effective care
Is able to support team development
