Experteer Overview In this role you operate as an autonomous advanced clinician within the East Sussex Community Persistent Pain Service, delivering expert assessment and person-centred care for adults with persistent musculoskeletal pain. You will work across community settings, using What Matters to You conversations and health coaching to empower patients to manage their pain and improve quality of life. You lead in clinical decision-making, risk assessment and MDT collaboration, contributing to service development and innovation within a NICE-guided care pathway. Pay / Benefits

  • Provide autonomous advanced clinical practice with accountable decisions Perform advanced triage to select appropriate care pathways Lead complex caseload management and MDT decision-making Collaborate with GPs, psychologists, pharmacists, and other partners to deliver coordinated care Contribute to service development, quality improvement, education, research and workforce development Develop personalised care plans using a biopsychosocial, person-centred approach Maintain accurate clinical records within governance standards Lead compassionate, trauma-informed care in consultations Deliver and evaluate group education and self-management programmes Present complex cases in MDTs and support transitions between services Tasks Significant autonomous experience in MSK/persistent pain Advanced clinical assessment, differential diagnosis and decision-making Experience of identifying red flags and escalation Multidisciplinary team collaboration and leadership experience Teaching, mentoring or supervision experience Professional registration with NMC, HCPC, GOsC or equivalent MSc in Advanced Practice and post-registration pain management training Knowledge of NICE guidance on persistent pain Ability to travel across East Sussex to meet role requirements Commitment to patient-centred, user-involved care Key requirements
