Full Job Description

We have an opportunity for an 3 year fixed term secondment to cover this Advanced Pharmacist role for Renal, Urology & Transplant Pharmacy Team. Practicing at an advanced level clinically, you will support the whole directorate but will develop and maintain a particular focus for Renal Dialysis patients.

The Renal Pharmacy team takes pride in delivering compassionate excellence to the diverse range of medical and surgical patients that the directorate is responsible for. This is an exciting and varied role, within an innovative and forward-thinking pharmacy team.

Main duties of the job

1. To participate in the provision of a safe, effective and efficient clinical pharmacy service to the Renal, Transplant and Urology Directorate.

2. To practice as an advanced clinical pharmacist and provide specialist review of renal and transplant patients, including prescribing as part of the multi-disciplinary team on wards and in clinics.

3. To have delegated responsibility for specific aspects of the renal pharmacy service as agreed with the Consultant Pharmacist Transplantation and Renal Services and Divisional Pharmacist Team Leader.

4. To promote and deliver medicines optimisation to ensure evidence based and cost effective use of treatments.

5. To provide specialised professional and technical education to enable the clinical units to meet high standards of patient care and safety.

6. To provide a framework for the safe, efficient and cost effective use of medicine resources within the specialist area, by the provision of education and training, procedures, policies and guidelines, and medicines usage information.

7. To develop and maintain highly specialist renal and transplant knowledge. To act as a role model within the specialist pharmacy and multidisciplinary teams to ensure a consistently high pharmaceutical service in line with national standards, Trust strategy and the NHS Plan.

You must have appropriate UK professional registration.
