Full Job Description
The post holder will be an autonomous practitioner who will be expected to take an active role in the respond, stabilise and make safe element of acute care pathways by delivery of advanced assessment, and the management and treatment of patients over 18 years of age patients with inflammatory bowel disease (IBD) referred to IBD Hot Clinic., The Advanced Practitioner role, within the IBD service, will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate. Working as part of a multidisciplinary team and across the GIMS single managed service, the post holder will be responsible for co-ordinating the delivery of urgent health care needs of people with IBD with an exacerbation of their disease within the IBD Hot clinic at risk of hospitalisation.
The post holder will provide specialised in-reach to the Emergency Department to triage, assess and input as an independent practitioner to patients with an exacerbation of their IBD. The focus will be to prevent unnecessary acute hospital admissions andenable patients to remain safely and supported at home and prevent premature admissions or readmissions.
