Full Job Description

Bro Pedr Medical Group is seeking an experienced and motivated Advanced Nurse Practitioner (ANP) to join our multidisciplinary team. This is an excellent opportunity to work within a supportive and forward-thinking primary care environment.

The successful candidate will work autonomously within their scope of practice, delivering high-quality, patient-centred care. The role involves assessing, diagnosing, and managing patients with acute and chronic conditions, supporting same-day demand, and contributing to long-term condition management.

You will work collaboratively with GPs and the wider team, providing care through face-to-face, telephone, video, and eConsult appointments. Responsibilities include triage, initiating and interpreting investigations, prescribing where appropriate, and making referrals to secondary care and other services as required .

The role also includes supporting care for care home residents, housebound patients, and those requiring palliative or end-of-life care, alongside involvement in immunisation programmes and health promotion., The Advanced Nurse Practitioner (ANP) will work autonomously and as part of the multidisciplinary team to deliver high-quality, patient-centred care. On a day-to-day basis, the postholder will manage a mix of same-day urgent appointments and routine clinics, assessing, diagnosing, and treating patients with acute and chronic conditions. This will include triage, prescribing (where qualified), requesting and interpreting investigations, and making referrals as appropriate.

The ANP will also contribute to the management of long-term conditions, support care for housebound and care home patients, and play a role in preventative care, health promotion, and immunisation programmes. Participation in duty/on-call rotas and supporting the wider clinical team with demand and patient flow will form part of the weekly workload.

The postholder will report clinically to the GP Partners and operationally to the Practice Manager, working closely with the wider clinical and administrative team.

We are a high achieving training practice offering a friendly working environment with a team of 4 GP partners, 4 Salaried GPs, an Advanced Nurse Practitioner, nursing team including HCAs and a friendly and efficient management, admin and reception team.

Hours: Full-time or part-timeSalary: Dependent on experience
