Advanced Nurse Practitioner

Working as an autonomous practitioner within an agreed scope of practice, you will assess and manage patients presenting with minor illness and appropriate undifferentiated symptoms.

You will take histories, undertake clinical examinations, develop differential diagnoses, request and interpret investigations, prescribe independently, agree treatment and follow-up plans, and refer or escalate patients where appropriate.

Alongside acute work, the post will include planned long-term-condition care. Depending on your experience and interests, this may include structured reviews, medicines optimisation, risk assessment and support for patients managing conditions such as diabetes, cardiovascular or respiratory disease.

The precise balance of acute and planned care will be agreed with the successful candidate, taking account of their experience, competencies and development interests.

The Fishponds Family Practice is looking for an experienced Advanced Nurse Practitioner and independent prescriber to join our friendly and established multidisciplinary team.

This is an opportunity for an ANP who enjoys the breadth of primary care and wants a role combining minor illness and acute assessment with the management of long-term conditions.

We are particularly keen to hear from clinicians with experience in diabetes care

Is a registered nurse with current NMC registration has completed an advanced clinical practice qualification at Master's level or has equivalent experience

Holds an independent prescribing qualification with appropriate NMC prescribing

Has recent experience of safe, independent prescribing

Has substantial experience assessing and managing minor illness, acute or undifferentiated presentations in primary care or a comparable setting

Is confident in history taking, examination, clinical reasoning and diagnostic decision-making

Can recognise red flags, manage clinical uncertainty and escalate appropriately

Has experience of long-term-condition management, including review, treatment planning and follow-up

Works well as part of a multidisciplinary team while being comfortable making clinical decisions

Experience and additional training in type 2 diabetes management would be particularly valuable. Experience with EMIS Web, QOF, teaching, supervision, audit or quality improvement would also be welcomed.

Fishponds Family Practice is a long-established teaching practice based on Beechwood Road in Fishponds, Bristol, caring for more than 12,500 patients.

Our multidisciplinary clinical team includes GP partners and salaried GPs, nurses, healthcare assistants and pharmacy colleagues, supported by an experienced Care Coordinator team.

What we offer

Salary from £27 per hour, depending on experience
