Advanced Nurse Practitioner

We are looking for a compassionate and experienced Advanced Nurse Practitioner to join our Home Visiting Service at Ashfield North PCN. This is a rewarding opportunity to make a real difference by providing timely, high-quality care to patients who are unable to attend GP practice settings across four practices in and around Sutton-in-Ashfield, serving a population of around 52,000. In this role, you will assess, diagnose, and manage a wide range of acute and chronic conditions through home visits and same-day consultations, delivering safe, effective, and person-centred care to those who need it most. You will play an important part in supporting GP practices by responding to triaged requests, helping to ease pressure on primary care services and improving access for housebound and care home patients. The role also offers the chance to contribute to reducing avoidable hospital admissions and supporting proactive care for patients within frailty pathways. As part of the Additional Roles Reimbursement Scheme (ARRS), you will be joining a wider multidisciplinary team across the PCN, working collaboratively to support integrated care and improve outcomes for patients and communities. You will work closely with GP practices, community services, and PCN colleagues, building strong working relationships while maintaining high standards of clinical practice, communication, and patient advocacy in line with organisational and professional requirements.,

• Act as an autonomous advanced practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients.
• Avoiding inappropriate admissions and maintain patients in their home environments with a safe package of care and referral to other agencies as necessary, including directly admitting patients to hospital when necessary.
• Responding to GP triage for home visiting both in the patients home and in care homes.
• Provide evidence based professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence and a proactive approach to care.
• To develop and lead new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice population.
