Our team thrives on collaboration, bringing together service managers, consultant psychiatrists, dedicated healthcare professionals, compassionate nurses, empathetic recovery staff, enthusiastic volunteers, and supportive peer mentors., The Advanced Nurse Practitioner Homelessness Outreach will provide advanced, autonomous clinical care to individuals experiencing homelessness across West Sussex, with a particular focus on addressing health issues related to substance misuse and alcohol dependency. Working flexibly between service hubs and outreach settings, the post holder will deliver comprehensive health assessments, urgent primary care including wound care/immunisations/BBV treatment pathways, and ongoing management of substance and alcohol related health conditions alongside coexisting physical and mental health needs. The role requires independent clinical decision making, nonmedical prescribing, and close collaboration with multiagency partners to respond effectively to the complex and often interrelated health and social needs associated with homelessness. Reducing health inequalities and improving access to timely, appropriate healthcare for this underserved population is a central priority of the role.
