Full Job Description

Bevan Medical Group are excited to welcome a new Advanced Nurse Practitioner to their established team. The successful candidate will serve a patient population of over 8,000 patients and be based in sites across the East Durham area. We are continually seeking new ways to support and improve local healthcare services to our patient population.

The successful candidate will be joining a growing multidisciplinary team including GP's, ANP's, Practice Nurses, HCA's, Clinical Pharmacists and PCN based roles.

We are committed to the ongoing development of our team, which includes mandatory and developmental training for all our staff. Team members are encouraged to highlight any training they feel would benefit them in the achievement of their role.

Main duties of the job

In your role, you will be based within the practice and will be required to provide some home visits to patients where appropriate.

The post holder will demonstrate courageous and critical thinking, they will be a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients of all ages within general practice.

The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required., The applicant should have excellent communication and interpersonal skills and be able to provide high quality, patient-centred care. You will work alongside the team delivering and developing care relevant to the needs of our patient population in conjunction with organisational, local and national priorities.

Our current team comprises of GP's, ANP's, Practice Nurse, Pharmacists and HCA's, supported by friendly and experienced administrative and management teams. We hold frequent practice, clinical, peer support sessions and encourage regular CPD sessions. We are proud to have been accredited as a training practice.

We have a patient list size of over 8,200 patients working from modern, purpose-built premises, based in County Durham. We are active members of Durham Coast Primary Care Network.
