Full Job Description

This is an exciting opportunity to play a pivotal role in providing high-quality, patient-centred care to our local community.

As an autonomous practitioner, you will manage undifferentiated and undiagnosed presentations, undertake clinical assessment, diagnosis, treatment, and evaluation of care, and work collaboratively with GPs, nurses, pharmacists, and wider health and social care partners.

The role is central to improving access, supporting same-day care, and delivering safe, effective management of both acute and long-term conditions.

You will utilise advanced clinical skills, independent prescribing, and evidence-based practice to optimise patient outcomes, contribute to service development, and support continuous improvement across Hwb Iechyd Cybi.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply., Provide autonomous, advanced clinical assessment, diagnosis and treatment for patients with a wide range of acute, urgent and long-term conditions. Manage a designated clinical caseload, including ordering and interpreting investigations, prescribing, and making appropriate referrals. Apply advanced clinical reasoning and critical thinking to support safe decision-making and high-quality patient care. Contribute to clinical leadership within the primary care team, supporting service development, pathway improvement and effective use of resources.. Provide education, supervision and support to nursing colleagues and the wider multidisciplinary team. Communicate effectively with patients, families and partner organisations, including managing sensitive and complex information. Participate in audit, research and continuous quality improvement activity to embed evidence-based practice. Ensure compliance with clinical governance, professional standards, and Health Board policies.

Previous knowledge of working within a primary care setting is desirable for this position.
