Advanced Mental Health Practitioner

The Newbury Community Older Adults Mental Health Team is looking for an enthusiastic, caring and highly motivated Memory Clinic ClinicalLead tojoin our friendly and innovative Community Team. Youwill bebasedatHillcroft House, West Berkshire Community Hospital where we have access to free parking facilities,astaff room andmost importantly afriendlyandsupportivewelcoming team.The serviceareworking flexibly withopportunities to workremotelyat times, frequency depending upon service need. The Newbury Older Adult Mental Health Service is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community.Thepost holder will be a senior practitioner with highly developedclinicalskillsworking withpatients in theOlder AdultsMental Health Specialism.He/shewill be offering highquality memory clinic assessments under the supervision of the service Drs.You will be supporting the smooth running of the service alongside the Memory Clinic Team Lead, offeringsupervision, education and highly specialist advice to colleagues and other agencies. The successful candidate must display a passion and understanding for working with OlderPeople,appropriate trainingand support will be provided as needed.You must be enthusiastic, caring and highly motivated to contribute towardsmaintainingstandards of high-quality care to our patients.Main duties of the job

• The post holder will offer high quality memory clinic assessments and whereappropriate deliverdiagnosis under the expert supervision of the memory clinic service doctors.
• The post holder will complete high standard assessment documentation and communication.
• The post holder willprovideexpert advice to patients, families, and carers in addition to the multi- disciplinary team,students,and community services.
• The post holder will support the Memory Clinic Team Lead to offer supervision, guidance, and advice to junior colleagues.
• The post holder will work alongside the Memory Clinic Team Lead to ensure the smooth effective and efficient running of the team.
• The post holder will be involved in the development of the memory clinic service, taking part in Quality Improvement projects, and supporting/assistingwith the process of MSNAP re-accreditation.
• The post holder will support the completion of service audits.
• The post holder will offer input to the Duty Worker rota.
