Full Job Description

As an ACP, you will work across all areas of the UCC, supporting a 7-day, 12-hour service. You will assess, diagnose, and treat a wide range of patients, drawing on advanced clinical skills to manage both differentiated and undifferentiated presentations. You will use expert communication, decision-making, and problem-solving skills to deliver timely, safe, and effective care.
You will play a key role in developing and enhancing our service, acting as a clinical expert and ensuring evidence-based practice is embedded throughout patient pathways. Working within nationally and locally agreed guidelines, you will independently assess, investigate, plan, and evaluate care while contributing to a positive, collaborative multiprofessional environment.
As part of your role, you will also support the clinical education and development of nursing, medical, and allied health colleagues, helping to build a skilled and confident workforce within urgent and emergency care.

Main duties of the job
The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role. The post holder will be guided by health, organisational, or broad occupational policies, but in most situations, the post holder will need to establish the way in which these should be interpreted and advise relevant stakeholders accordingly. They will be expected to work at an advanced clinical level within their level of competence and to work flexibly as part of the team.
The post holder will be expected to act as a practice leader, managing their own workload and working across professional, organisational, agency, and system boundaries to improve services and develop practice. They will participate in networks, locally, regionally, and nationally. The post holder will assess and manage risk and proactively challenge others about risk.
All Advanced Practitioner Roles must adhere to the Hywel Dda EAGLE Governance Framework for Advanced Practice. The following duties and responsibilities are designed around the Advanced Practice Pillars (Clinical, Leadership and Management, Education, Research) and guidance as outlined in the Framework for Advanced Nursing, Midwifery and Allied Health Professionals

We are looking for dynamic, motivated, and autonomous Advanced Clinical Practitioners (ACPs) to join our expanding Urgent Care Centre (UCC) at Prince Philip Hospital. This is an exciting opportunity to be part of a progressive, patient-focused service providing high-quality care for individuals presenting with urgent but non-life-threatening conditions., The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.
