Full Job Description
Local Care Direct is a non-profit Social Enterprise operating across 24 hours a day, 7 days a week, 365 days a year, to provide various primary and urgent health care services across West Yorkshire.,
- The advanced clinical practitioner will support the Urgent Community Response (UCR) services mission to deliver interventions within a 2-hour timeframe, either through face-to-face visits by partner clinicians or remote management.
- The advanced clinical practitioner will be an integral member of the clinical team to deliver high-quality patient care, that involves utilising technology to conduct remote consultations for patients referred into the service.
- The advance clinician practitioner will have a strong interest in complex triage and is skilled in remote consultations and confident in managing acutely unwell patients. The advanced clinician practitioner will strive to make a real difference by helping patients stay independent and avoid unnecessary hospital admissions.
