Advanced Clinical Practitioner-Stroke

The candidate will possess a completed relevant MSc in advanced practice, have extensive clinical experience in Stroke care, that will enable them to work as part of the medical team seeing adult stroke patients. This is an exciting time for development and expansion at Darent Valley, and you will work within a dynamic multi- disciplinary team and under the supervision of the Clinical lead for stroke to enhance the patient experience through the delivery of excellent patient-focused care. A commitment to the advancement of stroke care, together with the knowledge and expertise of change management are also required. As an ACP you will act as a leader within the service and role model to junior colleagues, The post holder is required to deliver an advanced level of clinical practice within the field of Stroke care. Working within local and nationally agreed protocols and guidelines, the ACP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of Stroke patients. They will work alongside and support the education and development of both nursing and medical colleagues.- ACPs are able to look after patients with a wide range of pathologies from the life threatening to the self-limiting, critically ill and injured with expertise in resuscitation and skilled in the practical procedures The ACP practises both autonomously and in partnership with the multidisciplinary team under the clinical supervision provided byan Clinical lead for stroke.. The ACP complements the Middle Grade roster and the nursing rota by acting as a senior resource for nurses.The ACP will deliver 90% direct clinical care, with the remaining 10%
dedicated to their own professional development, which may include audit, research and development.
