Full Job Description
As an Advanced Clinical Practitioner reporting to the Consultant Practitioner, you will provide an autonomous ACP service in Community Hospital inpatient settings. You will conduct advanced assessments, diagnose, initiate treatment, arrange investigations and coordinate timely, patient centred care, promoting shared decision making and avoiding unnecessary acute transfers.
You will provide clinical and professional leadership to nurses and the multidisciplinary team, proactively managing clinical, staffing and performance issues. Working closely with Community Team Key Workers, you will support safe discharge, monitor care quality, use resources effectively and contribute to budget management.
The role includes responding to incidents, complaints and safeguarding concerns, ensuring learning and service improvement through audit, feedback, research and implementation of national guidance. You will maintain standards in information governance, health and safety, training, appraisal and revalidation, promote a learning culture, support service development, attend required meetings and provide cover for peer ACPs when needed.
