Advanced Clinical Practitioner - Manchester Community Response Teams

We are seeking an enthusiastic and experienced ACP to join the South Manchester and Trafford Urgent Care Hub, supporting Urgent Community Response and Virtual Ward/Hospital at Home services.

The post holder will provide advanced clinical assessment and decision-making for patients with acute and complex health needs in their own homes and community settings. Responsibilities will include comprehensive assessment, history taking, diagnosis, treatment planning, prescribing and evaluation of care, with a strong focus on supporting people to remain safely at home and preventing avoidable hospital attendance or admission.

Working within an innovative multidisciplinary team, the ACP will collaborate across primary, secondary, community and social care interfaces to coordinate urgent and responsive care for people at risk of hospitalisation, as well as those requiring enhanced support following discharge or through Hospital at Home pathways.

Referrals may be received from NWAS, primary care, Emergency Departments, acute hospital teams and other community services. The post holder will work flexibly across South Manchester and Trafford, contributing to Urgent Community Response and Virtual Ward/Hospital at Home pathways and helping shape the continued development of an integrated Community Urgent Care Hub.

Main duties of the job

The post holder will provide specialist triage, assessment and clinical intervention as an autonomous practitioner, developing effective relationships with members of the multidisciplinary team to support patients across a range of healthcare settings, including Manchester Integrated Neighbourhood Teams, Intermediate Care Services, Out of Hours Services, community beds and primary care.

The role will focus on preventing avoidable hospital admissions, enabling patients to remain safely and independently at home, and reducing premature admission to long-term care. The post holder will support patients to proactively manage and anticipate future health needs by developing personalised care plans in collaboration with GPs, medical staff, Allied Health Professionals, nursing and social care teams.

The ACP will promote patient independence, self-management and proactive management of long-term and acute health needs, ensuring appropriate escalation and follow-up where required.

The post holder will lead the development, delivery and evaluation of high-quality, evidence-based, patient-centred interventions in partnership with relevant clinical teams. They will contribute to integrated working across community services and support the delivery of the objectives of the Community Urgent Care Division and Manchester Local Care Organisation, ensuring care is coordinated, responsive and focused on achieving the best possible outcomes for patients in their own homes and communities.
