Advanced Clinical Practitioner - Community

We are seeking an enthusiastic, proactive Advanced Clinical Practitioner with experience working in the community.

The role involves working within a community nursing team, supporting the team to offer a high level of care to patients in the place they call home.

The post holder will be part of the Urgent Community Response service, visiting patients who are acutely unwell and assessing their clinical and functional needs to try to reduce unnecessary hospital admissions. They will also be involved in the development of the virtual wards, working closely with hospital colleagues., The post holder will use their skills within the four pillars of Advanced Practice to deliver a very high level of care to complex patients in the community.

We welcome applications from Graduate visa holders; it is important to note that you must have a minimum of 6 months before this expires at the start of your employment for training and operational reasons., Please be aware that this post requires an Enhanced DBS (Disclosure and Barring Service) check with barred lists, the cost of which is payable by the successful candidate. If you have lived or worked abroad for 6 months or more in the last 5 years you will need to obtain an overseas police clearance.
