Advanced Clinical Practitioner - Asylum Assessment Service

Are you an Advanced Clinical Practitioner?Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs/ ANPs to join our bank workforce with opportunities within our Community Services based within the Asylum Service.,

• Assess, diagnose, plan, implement and evaluate treatment, interventions and care for people presenting with undifferentiated, undiagnosed, acute, minor, complex or vulnerability-related health needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.
• Diagnose and manage both acute and chronic conditions, integrating drug and non-drug-based treatment methods into a management plan.
• Ensure any patient requiring referral for an identified health, social care or safeguarding need is referred or escalated through the appropriate pathway within the required timeframe.
