Full Job Description
We are seeking a confident, autonomous Advanced Clinical Practitioner to work at Ashton GP Service. You will play a key role in delivering safe, effective and patient centred care within a supportive multidisciplinary team. Your responsibilities will include:
- Managing acute and routine presentations across both sites
- Undertaking clinical assessments and diagnostics
- Prescribing independently
- Supporting continuity of care for patients with long term conditions
- Working collaboratively with GPs, nurses and the wider MDT
- Contributing to service development and quality improvement What We Offer

Advanced Clinical Practitioner £55,157 to £68,544 per annum pro rata, yearly salary increments, NHS Pension provider. An exciting opportunity to work in a supportive and community focused practice. We have an exciting opportunity for an Advanced Clinical Practitioner to join our team and work in a well established practice, Ashton GP Service. You will be supported by strong clinical leadership and a collaborative multidisciplinary team. The hours for this role are 15 hours per week, to be worked on Wednesday and Friday between 08:00 and 18:30.
