Full Job Description
Jorvik Gillygate Practice is looking for an experienced and enthusiastic Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our established multidisciplinary team. We are particularly interested in applicants who are qualified independent prescribers, although we would be happy to consider an experienced candidate who is currently completing, or committed to completing, their prescribing qualification. This is a varied and autonomous clinical role, working alongside our GP, nursing, pharmacy and wider multidisciplinary teams to assess, diagnose, treat and manage patients presenting with a broad range of acute and ongoing health needs., The successful candidate will work autonomously within their professional competence and scope of practice, with appropriate clinical support available., The successful candidate will work autonomously within their professional competence and scope of practice, with appropriate clinical support available. The role will include:
- Assessing patients presenting with acute, undifferentiated and complex conditions
- Taking comprehensive clinical histories and undertaking appropriate physical examinations
- Formulating differential diagnoses and clinical management plans
- Prescribing safely and appropriately within scope of practice
- Requesting, interpreting and acting upon diagnostic investigations
- Managing acute and same-day presentations
- Supporting the management of long-term conditions
- Providing preventative and proactive care
- Undertaking home visits where clinically required
- Referring to secondary and community services where appropriate
- Working closely with GPs and the wider multidisciplinary team
- Maintaining accurate and contemporaneous clinical records
- Supporting patients with self-management, health promotion and prevention
- Contributing to clinical audit, quality improvement and service development For an applicant with an interest in Heart Failure, there may be opportunities to support and further develop this area of care within the practice. Advanced Practice The role reflects the four pillars of advanced practice: Clinical Practice providing autonomous, evidence-based assessment and management of patients. Leadership contributing to clinical pathways, service development and multidisciplinary working. Education maintaining professional development and supporting the learning and development of colleagues and trainees. Research & Quality Improvement contributing to audit, evidence-based practice and service improvement.
- Registered healthcare professional with current NMC, HCPC or other appropriate professional registration
- Advanced clinical practice qualification at Masters level or equivalent relevant advanced practice qualification
- Significant post-registration clinical experience
- Experience of autonomous clinical assessment and decision-making
- Ability to assess and manage undifferentiated presentations
- Strong clinical reasoning and diagnostic skills
- Excellent communication and interpersonal skills
- Ability to work effectively both independently and as part of a multidisciplinary team
- Experience within primary care, urgent care or a comparable clinical environment, Independent prescribing qualification
- Previous experience working as an ACP or ANP in primary care
- Experience managing acute and same-day presentations
- Experience of long-term condition management
- Desirable
- Interest or experience in Heart Failure
- Experience of clinical leadership or service development
- Experience supervising or mentoring other clinicians or learners
- Evidence of audit or quality improvement work
- Additional relevant clinical qualifications or specialist interests
