Learning Enterpri, We are looking for an experienced and motivated Advanced Clinical Practitioner (ACP) to join the HCRG Care Group Inpatient Unit team. This is an exciting opportunity to work within a supportive multidisciplinary environment, delivering high-quality, patient-centred care across our community inpatient services. Based at Paulton Memorial Hospital, you will provide advanced clinical assessment, diagnosis, treatment and management for patients requiring inpatient care. Working autonomously within your scope of practice, you will play a key role in supporting patient recovery and facilitating safe, effective care. You will provide advanced clinical care to patients within our community inpatient rehabilitation services, primarily based at Paulton Memorial Hospital, with occasional cross-cover support at St Martins Hospital. You will assess newly admitted patients, formulate diagnoses and management plans, request and interpret investigations, and deliver high-quality, patient-centred care that
