As an Advanced Clinical Practitioner, youll be part of our valued team in our Hospital at Home service based at in either Swindon or BaNES localities, you will provide acute hospitallevel care to patients across the locality. You will assess newly admitted patients, formulate diagnoses and management plans, request and interpret investigations, and deliver high-quality, patient-centred care that supports rehabilitation and recovery. Working autonomously within your scope of practice, you will manage complex clinical presentations and respond to acute health concerns that arise during a patient's inpatient stay. You will work closely with Consultant Geriatricians, specialist doctors, nursing teams, and therapists, contributing to ward rounds, multidisciplinary team meetings, and decision-making to ensure safe, effective, and coordinated care.
