Full Job Description

GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is working in partnership with the local Primary Care Networks (PCNs) to support the delivery of the NHS England Direct Enhanced Service (DES) contract., This is a full-time position (37.5 hours per week), covering Canvey PCN, which consists of the following practices, Deliver advanced clinical practice using expert knowledge and skills to provide holistic patient care.

Assesses diagnoses and treats patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.

Assesses diagnoses and treats patients in surgery or own home who require acute medical attention and refer to appropriate agency.

Refers patients as appropriate to other members of the multi-disciplinary team, secondary care and to other statutory and voluntary organisations.

Interprets a range of diagnostic tests and routine clinical procedures.

Extended and supplementary prescriber.

Provides highly specialised advice concerning care normally beyond the scope of normal nursing practice e.g., establishes differential diagnoses, management/treatment plans and makes referrals as appropriate.

Identify patients at risk of frailty using clinical indicators (e.g., Rockwood score, dementia diagnosis) and ensure appropriate care planning in partnership with the PCN team.

Ensure accurate registration of all adults living with frailty or dementia on the eFraCCS frailty register.

Coordinate and implement care plans aimed at reducing avoidable hospital admissions and unnecessary primary care attendees.

Support the weekly care home ward rounds, ensuring continuity and quality of care for residents.

You will play a key role in shaping services, influencing change, and improving outcomes for patients across the network.

Competitive salary (depending on experience, pro rata for part-time).

37.5 hours per week (part-time options available)

Permanent Contract.

30 days annual leave plus bank holidays (pro rata for part time).
