Full Job Description

We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join our Primary Care Network (PCN). The ACP will provide high-quality, patient-centred care, working autonomously to assess, diagnose, treat and manage patients with a wide range of acute, chronic and complex health conditions.

The successful candidate will undertake advanced clinical assessments, develop and implement treatment plans, prescribe medication where appropriate, and order and interpret diagnostic investigations. They will work closely with GPs and the wider multidisciplinary team to ensure safe, effective and integrated care for patients.

The role also includes providing follow-up care, supporting patients to manage their health, promoting health education and wellbeing, and contributing to the continuous improvement of services. The ACP will support clinical governance, participate in audits and quality improvement initiatives, maintain accurate patient records, and ensure compliance with professional, organisational and regulatory standards.

In addition, the post holder will act as a clinical resource within the team, sharing expertise, supporting colleagues, and contributing to service development. As part of the NHS England Additional Roles Reimbursement Scheme (ARRS), we are seeking a skilled and motivated Advanced Clinical Practitioner (ACP) to join our Primary Care Network (PCN) team. The Advanced Clinical Practitioner (ACP) will be able to assess and treat varied clinical cases, having advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis. Advanced communication, problem-solving and decision-making skills will be used to initially assess and provide on-going patient care. Working within local and nationally agreed protocols and guidelines, the ACP exercises independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients.
