Advanced Clinical Practitioner

We have an exciting opportunity for an Advanced Clinical Nurse Practitioner [ACP] to join our Memory Service multi disciplinary team. This is a primarily clinical role focused on the assessment, diagnosis and management of patients presenting with a cognitive impairment., As the ACP will work autonomously to deliver expert clinical intervention and care planning, through your specialty knowledge and skills, providing an independent review of treatment plans and advising and supporting the B6 Assessors in the team. You will work closely with the team manager to provide clinical leadership, advice and training to the team and be responsible for ensuring the delivery of evidence based practice within the service, bringing the four pillars of advanced practice into working practice to help maintain and improve the quality of the service provision. at the MDT meetings.
