Full Job Description

We are seeking an experienced, autonomous and patient-focused Advanced Clinical Practitioner (ACP) to join our innovative multidisciplinary team. This is an exciting opportunity to play a key role in delivering same-day care across our Urgent Care Service, Minor Illness Clinics and Clinical Allocation Hub, supporting patients to access the right care, from the right clinician, at the right time. Working as an autonomous practitioner within your scope of practice, you will provide advanced clinical assessment, diagnosis, treatment and prescribing for patients presenting with urgent and acute healthcare needs. You will undertake face-to-face consultations within our urgent care clinics, deliver telephone and digital consultations, and carry out home visits where clinically appropriate., The Advanced Clinical Practitioner is a key member of The Middlewood Partnership multidisciplinary team. Working autonomously and within their scope of practice, the post holder will:

  • Assess, diagnose, investigate, treat and manage urgent and acute presentations.
  • Prescribe appropriately and coordinate referrals and multidisciplinary care as required.
  • Undertake clinical triage and assessment of patient enquiries and requests within the Clinical Allocation Hub.
  • Support patients to access the most appropriate healthcare professional or service in a timely manner.
  • Provide expert clinical advice and assessment via telephone, digital platforms and face-to-face consultations.
  • Deliver safe, evidence-based and patient-centred care.
  • Contribute to service development, quality improvement and clinical effectiveness initiatives.
