Advanced Clinical Practitioner

Are you an experienced Advanced Clinical Practitioner, have you experience or an interest in General Practice and looking for a new challenge? An exciting opportunity has arisen for an Advanced Clinical Practitioner to join our dedicated, dynamic, and friendly team at Marshalls Cross Medical Centre based within St Helens hospital. You may also work within the community including case managing and providing holistic medical care to patients within a busy care home. You will be an integral part of Mersey & West Lancashire's clinical team supporting our GP surgery and care home residents. You will clinically support patients (both adults and children) with acute and urgent health conditions and with an emphasis on prevention of hospital care and or admission. You will further support our team with professional development, supervision and ensuring the most up to date evidence-based practice is implemented promoting the best patient experience and outcomes. Previous experience within a Primary Care and/or Community setting is essential for this role., The post holder will work autonomously as a senior nurse within the Primary Care/Community environment, reviewing patients with undifferentiated and undiagnosed health problems; triage, taking comprehensive histories, undertaking clinical examinations, diagnosing, planning & implementing car, prescribing medication and making referrals where appropriate. • Occasional requirement to exert moderate physical effort for short periods during a given time when moving and transferring patients using hoists and other lifting aids
• Required to move patients on trolleys and in wheelchairs
• Required to concentrate when carrying out care. Work is generally predictable but occasional requirement to deal with unpredictable emergency situations

• Significant demonstrable experience of working in an acute care setting some of this must include working at a senior level managing and leading teams and this will be assessed at interview
• Ability to demonstrate specialist expertise underpinned by theory through CPD
• Excellent clinical reasoning /clinical decision making skills
• Evidence of involvement in the development of programs of care, protocols and guidelines
• Knowledge of Clinical risk management and Governance
