Advanced Clinical Practitioner

The Advanced Clinical Practitioner is an autonomous senior clinical leadership role. The post holder will have undertaken post-graduate training in Advanced Clinical Practice, while having consolidated your advanced-level knowledge and skills within your scope of practice. You will have gained a high-level of training and practice-based experience through the four pillars of advanced practice (Clinical Practice, Leadership, Education and Research). The post holder will deliver, and lead on the delivery of, high quality safe and contemporary care, ensuring consistently high standards of practice and clinical excellence, including: promoting population health, improving personal outcomes, contributing towards service evaluation, while leading and developing research and quality improvement initiatives. The post holder is expected to place the well-being and safety of service-users/patients above all other considerations, with responsibility to recognise and work within the limits of your professional competence. It is the joint responsibility of the Post Holder, in collaboration with your Operational Manager, to undertake the ongoing assessment of competence, ensuring this is monitored and recorded in line with clinical governance and organisational policy., This is a senior, autonomous clinical leadership role working across the four pillars of advanced clinical practice: clinical care, leadership, education, and research The post holder will: Provide advanced assessment, formulation, and care planning for people with complex mental health needs Lead on safe, effective, and evidence-based interventions, including prescribing (where qualified) Work across multidisciplinary teams and partner agencies to deliver coordinated, integrated care Contribute to service development, quality improvement, and pathway redesign Support and supervise staff, including trainee ACPs, while promoting a culture of learning and reflective practice Demonstrate the knowledge and skills to articulate comprehensive assessment, formulation and care planning, verbally and in written form. Act with inclusivity, initiative and creativity to incorporate strength-based principles to support people to address their individual needs. Uses communication, negotiation, conflict management skills; requiring empathy, reassurance and persuasion, where there are significant barriers to change. Demonstrates the advanced skills and knowledge to undertake comprehensive person-centred history taking, assessment and formulation; considering biological, psychological, social and spiritual perspectives. • To be a qualified Healthcare Professional (i.e. Nurse, Social Worker or Occupational Therapist)
• To hold current professional registration with respective professional body
• To hold a PGDip in Advanced Clinical Practice or equivalent credentialed programme
• Independent prescriber
• Portfolio of evidence for advance clinical practice consolidation
Desirable
• MSc in Advanced Clinical Practice
• Evidence of education/training in specialist area (i.e. therapies, interventions, service-specialism)
  
  Extensive experience of leading the delivery of care for a diverse population
• Experience working within healthcare services at an advanced level of practice
• Advanced level understanding of specialist-specific disorders and their corresponding evidence-based interventions including LD and neurodiversity
• Contemporary knowledge of relevant legislation and healthcare policies
• Applied knowledge of safeguarding, inclusion and equality
• Experience of working in working age adult community/inpatient mental health services, Experience of writing for publication and/or presenting to professionals and the public, Advanced skills in undertaking clinical & risk assessments, specific to the area of practice
• Advanced level written and verbal communication skills
• Effective self/time management skills to practice as an independent practitioner
