Full Job Description
As our Clinical Streaming Hub and Integrated Urgent Community Care Teams continue to evolve and grow, we are seeking motivated, autonomous ACPs who thrive on clinical challenge, complex decision-making, and system-wide working., This is not a traditional community ACP post. You will make autonomous, high-level clinical decisions within your scope of practice, confidently managing risk to keep patients safely at home wherever appropriate. This role spans:
- Hospital at Home & Digital Ward - clinical responsibility for patients safely managed at home
- Urgent Outreach/ Crisis Response - undertaking assessments in patients' homes, care homes and community settings
- Clinical Streaming & ED Interface - working with acute physicians and ED teams to safely redirect and manage patients
- Ambulance Triage & Avoidance - identifying patients suitable for community-based assessment to prevent unnecessary conveyance, Undertake complex clinical decision-making across community, intermediate and acute care interfaces.
- Deliver care within the Clinical Streaming Hub, Hospital at Home, Digital Ward, Urgent Outreach/Crisis Response and associated urgent care services.
- Assess and manage patients within their own homes, care homes, community settings,clinical environments.
- Identify patients suitable for admission avoidance pathways and safely support care closer to home.
- Work collaboratively with Primary Care, ED, Acute Medicine, Welsh Ambulance Services, community teams to ensure patients receive care in the right place and time.
- Provide senior clinical advice, leadership and support to multidisciplinary colleagues.
- Contribute to service development, pathway redesign, quality improvement and innovation initiatives.
- Support education, supervision and professional development of colleagues and learners.
- Maintain advanced clinical competence, professional registration and prescribing practice in line with national and local requirements

- Motivated to shape new ways of delivering urgent care
- Passionate about admission avoidance and improving patient flow A prescribing qualification (or commitment to obtain) is essential in line with the attached JD.
