Full Job Description

  • The advanced clinical practitioner will support the Urgent Community Response (UCR) services mission to deliver interventions within a 2-hour timeframe, either through face-to-face visits by partner clinicians or remote management.
  • The advanced clinical practitioner will be an integral member of the clinical team to deliver high-quality patient care, that involves utilising technology to conduct remote consultations for patients referred into the service.
  • The advance clinician practitioner will have a strong interest in complex triage and is skilled in remote consultations and confident in managing acutely unwell patients. The advanced clinician practitioner will strive to make a real difference by helping patients stay independent and avoid unnecessary hospital admissions.

    Evidence of a range of clinical experience, working independently in a healthcare role.
  • Evidence of experience of triage, by telephone or face-to-face

  • Minimum of 1 year experience in practice post qualification.
  • Experience of working in primary/urgent care.

  • Experience of telephone/video remote consultation., One of the following: 1st level Registered Nurse or equivalent professional qualification/registration with a professional body, MSc/Post Graduate diploma in Advanced practice, Paramedic HPC registration MSc/Post Graduate diploma in Advanced practice or Pharmacy GPhC Registration MSc/Post Graduate diploma in Advanced practice
  • Independent & Supplementary prescribing qualification
