Full Job Description
GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is working in partnership with the local Primary Care Networks to support the delivery of the NHS England, Direct Enhanced Service (DES) contract., We are looking to recruit a PCN Advanced Clinical Practitioner to work alongside member practices with Canvey Island Primary Care Networks (PCN). The role is part time working 9.5 hours on a Friday This role will be a core member of the multidisciplinary team, working alongside other professionals, including Clinical Pharmacists, Senior Paramedics, pharmacy technicians, physician associates, occupational therapists, mental health practitioners and Care Coordinators. You will be involved in PCN developments and have the ability to influence change and improve care for patients as well as the opportunity to lead on specific clinical based projects within the network.Main duties of the job This role will be working with patients within the network seeing people with undifferentiated and undiagnosed conditions. The post holder will be supported by the Clinical Team Leader. The successful post holder will work autonomously within their professional boundaries to: Undertake comprehensive health assessments, initial history taking, clinical assessment, physical examination, diagnosis, treatment and evaluation of their care Use advanced clinical decision making and assessment skills, coordinate care planning and initiate onward referrals as appropriate Demonstrate sound safe clinical decisionmaking. Be an independent non-medical prescriber.
Canvey Island PCN is made up of six GP surgeries covering a population of approximately 40,000 patients. Our PCN serves an area with an interesting population with a varied case mix.
