This is an exciting opportunity to join South West Yorkshire Partnership Teaching NHS Foundation Trust as an Advanced Clinical Pharmacist supporting mental health services at Cheswold Park Hospital, Doncaster. Cheswold Park Hospital is part of the Trusts Forensic Psychiatry services and is a medium secure hospital. This is a brand new post so if you're looking for a role with real influence, professional freedom and the chance to shape how pharmacy supports mental health care, this post offers exactly that.

You'll work with a high level of independence, managing your own workload and leading projects that directly improve medicines optimisation and patient experience. As a senior member of the local pharmacy team, you'll provide advanced mental health medicines expertise, offer clinical leadership, and oversee the management and supervision of pharmacy colleagues.

Your clinical work will include delivering specialist pharmacy services to inpatients at Cheswold Park Hospital and developing the pharmacy service at the site which has until recently been provided by an independent provider.

As part of the wider South West Yorkshire Partnership NHS Foundation Teaching NHS Trust pharmacy service, you will have access to support, training, & supervision from experienced mental health pharmacy professionals. You will also play a key role in trustwide medicines optimisation initiatives & contribute to delivering our new Medicines Optimisation & Pharmacy Strategy.

Main duties of the job

The role of the post holder will include provision of direct patient care, carrying out patient consultations and medication counselling, complex medication reviews, rationalizing and optimizing medicines for mental health conditions as well as concurrent medicines for those with long term physical health conditions, prescribing where appropriate and qualified.

There will also be a role to support other health care professionals in relation to mental health medicines including responding to queries and education and training. This role will provide clinical supervision to other members of the mental health pharmacy team.

Trust wide pharmacy activities which the role will lead or provide input into may include

Clinical audit

Quality improvement projects

Research and development

Policy development and review

Develop training packages

Manage key performance indicators in the locality

Supporting good prescribing practice and supporting nursing practice in respect of safe and secure handling, questioning and challenging where appropriate

Budget analysis and monitoring

Monitoring of high risk and high-cost medicines at a service level.

Have responsibility for a specific trust wide work stream
