Full Job Description
Are you an experienced palliative care nurse with a passion for innovation and continuous improvement? At Marie Curie, we are excited to invite highly driven and enthusiastic individual to join our new service at the Royal Stoke University Hospital. In this role, you will be responsible for actively identifying patients in the Emergency Department who are likely in their last year of life, providing specialist palliative care and facilitating their prompt discharge to appropriate care providers, aligned with patients preferred place of care., What you will be doing:
- Lead on clinical assessments and care planning
- Provide advanced symptom control and communication support
- Coordinate with community and hospital palliative care services to facilitate rapid discharge home/preferred place of care
- Supervise Healthcare Assistants, mentor emergency department staff, and support team learning You will also manage nursing services for individuals with terminal illnesses, supporting their loved ones, and collaborating with health professionals. As a vital member of our multidisciplinary team, you will offer expert advice and support, managing a caseload of patients with complex needs while ensuring the highest standards of care.
