Adults Care & Support Social Worker - Rhondda Cynon Taff Council

Both roles are based at Trecynon, Aberdare, for the team day, with hybrid working offered and attendance required on site one day per week for your allocated team day. The teams support adults aged 18+ with a range of needs including physical disabilities, chronic illness, learning disabilities, and mental health conditions. You will manage a varied and rewarding caseload, undertaking complex assessments and interventions while working closely with families and partner agencies.   Social Work qualification and Social Care Wales registration
Experience within Adults Services Strong assessment, communication, and report writing skills Knowledge of safeguarding procedures and the Mental Capacity Act 2005 A proactive, organised, and compassionate approach to practice Commitment to strengths-based and person-centred working Experience or understanding of Continuing Health Care and Care & Treatment Planning under the Mental Health Measure (Wales) 2010 would be advantageous.
