Full Job Description
As Adult Social Care Lead for Prevention, Wellbeing and Community Connection, you will lead the development of interventions that draw on community assets, partnerships and strengths-based practice from first contact. You will lead a multidisciplinary first response approach, ensuring timely assessment, effective triage and a consistent right response first time. You will champion a Home First philosophy that supports effective hospital discharge and recovery, strengthen access to reablement as a preventative intervention, and drive the expansion of technology enabled care across Dudley to enhance independence, safety and wellbeing.,

We are looking for a qualified Social Worker or Occupational Therapist, registered with Social Work England or the HCPC, with significant senior management experience in a complex, multidisciplinary organisation. You will bring demonstrable experience of prevention, reablement or hospital discharge pathways, ideally including strengths based practice, community partnership working or technology enabled care, together with a track record of leading service transformation, managing budgets and demand, and turning strategy into effective operational delivery. You will be a credible, resilient and politically astute professional, an effective communicator and influencer who can build trust across organisational boundaries and with elected members.
