A new and exciting opportunity has arisen in our newly redesigned Nursing care model within the Hillingdon Care Connection Team (CCT). We are in need of someone to join our team as a Senior Administrative Co-Ordinator, working on a permanent basis. The CCT comprises of a Guided Care Matrons (GCM), GPs, Health Care Support Workers (HCSW), Wellbeing Support Assistants and a Senior Administrative Co-Ordinators. Together, they provide active case management for adult patients with long-term conditions in order to prevent unnecessary hospital admissions. Band 4 Senior Administrative Co-Ordinator work closely with people who use our Services, their families, friends and carers and are an important contributor in how people experience our services. Band 4 Senior Administrative Co-ordinators are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by: · keeping the people who using our services as safe as possible by recognising the limits of the Senior Administrative Co-Ordinator role and working closely with GCMs. · contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures when implementing care plans · ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of care plans We believe that the best health care is delivered by multi-disciplinary teams working well together and in partnership with other teams and services to provide seamless care. Band 4 Senior Administrative Co-Ordinators contribute to the team in achieving the team's objectives by working closely with and drawing support from GCMs and other health professionals.   • Monitor referrals andadd into new referral caseload on SystmOne (S1) and EMIS.
  • Support with onward referrals and carefully avoiding duplication.
  • Monitor caseloads and liaise with GCMs when patients have not had recent contact, to ensure caseloads remain active and up to date.
  • Diary management for HCSWs.
  • Participate in team handovers
  • Support with administrative actions- daily handovers.
  • Keep a daily log of clinical handovers and document actionsusing Situation Background Assessment Recommendation (SBAR) tool.
  • Attend team meetings and take minutes.
  • Patient searches - use platforms such asEMIS, WSIC and hospital admission emails topull data and share with GCMs.
  • Once the GCM has completed Self Management Plan (SMP), Senior Administrative Co-Ordinator will print the SMP for the GCM todeliver,discussand get it signed.
  • Ordering of supplies from NHS Oracle.
  • Monitor Generic email addresses.
  • Register new patients referredinfrom thehuddle meetings.
