Full Job Description

Join our dynamic Acute & Emergency Medicine pharmacy team and make a real impact at the front door of care. As a Band 7 Clinical Pharmacist, you'll work across the Emergency Department, Acute Medical Unit, Same Day Emergency Care (SDEC), Frailty in-reach and Urgent Treatment Centres--supporting safe, effective and timely treatment for acutely unwell patients. You'll provide high-quality clinical pharmacy input, optimise therapies, support admission avoidance (particularly in frailty), and help facilitate safe discharge directly from ED. Working closely with the multidisciplinary team, you'll play a key role in improving patient flow and medicines safety in a fast-paced environment. This is an excellent development role with structured support from senior pharmacists and our Principal Pharmacist ACP. You'll have opportunities to grow your skills in:

  • If you're enthusiastic, motivated and ready to develop specialist expertise in emergency care, we'd love to hear from you.Main duties of the job Deliver safe, effective clinical pharmacy services across ED, AMU, SDEC, Frailty in-reach and UTC. Provide medicines optimisation during ward rounds and MDT discussions to support timely clinical decisions. Use (or work towards) independent prescribing to optimise therapies and support admission avoidance. Contribute to quality improvement, audit and service development within Acute & Emergency Medicine. Support day-to-day clinical pharmacy activity, including medicines reconciliation, review and discharge planning. Provide clear, patient-focused medicines advice to support safe discharge and virtual ward pathways. Work with senior pharmacists to ensure safe prescribing, guideline adherence and effective medicines governance.
