Full Job Description
The post holder will work as an integral member of the multidisciplinary team within a Community Hospital rehabilitation setting, supporting patients to regain and maintain their independence through meaningful activity promoting reablement, rehabilitation and recovery.
Working under the guidance of registered professionals but without direct supervision, the post holder will provide practical assistance and implement rehabilitation-focused interventions that promote physical, cognitive, and emotional wellbeing. This includes facilitating creative, stimulating, and functional activities tailored to individual goals and care plans.
The post holder will work as an integral member of the multidisciplinary team within a Community Hospital rehabilitation setting, supporting patients to regain and maintain their independence through meaningful activity promoting reablement, rehabilitation and recovery. Experience with dementia patients is desirable.
