Full Job Description

In our newly built and invested FSDEC, working at an advanced level of practice, the ACP will deliver comprehensive and compassionate care, initiate evidencebased management plans, and facilitate safe sameday discharge where appropriate. The role supports system flow, reduces unnecessary admissions, and improves outcomes for frail patients through collaborative, multidisciplinary, and personcentred care., The ACP will lead on the delivery of Same Day Emergency Care to those at point of need encompassing all four pillars of Advanced Clinical Practice.

Advanced Clinical Practice

Undertake autonomous, advanced assessment of patients presenting to Frailty SDEC,

Diagnose, treat, and manage complex acute and longterm conditions associated with frailty, including delirium, falls, polypharmacy, functional decline, and dementiarelated presentations.

Leadership & Management

Act as a clinical leader within Frailty SDEC, promoting best practice and high standards of care.

Support clinical decision making during periods of high demand, acting as a senior point of escalation.

Participate in workforce planning, rota development, and supervision of ACPs, trainees, and junior clinicians.

Promote a culture of safety, openness, and continuous improvement., Provide clinical supervision, mentorship, and teaching for ACP trainees, nurses, AHPs, junior doctors, and students.

Contribute to education programmes relating to frailty, CGA, urgent care, and SDEC pathways.

Support competency development and assessment within the frailty workforce.

Research, Audit & Quality Improvement

Participate in and lead audit, research, and quality improvement projects relevant to frailty and urgent care.

Contribute to local and national frailty initiatives and networks where appropriate.
