Full Job Description

Barnsley Hospital NHS Foundation Trust is seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join our Surgical Same Day Emergency Care (SDEC) service. This role is central to delivering high-quality, patient-centred care for individuals presenting with acute surgical conditions, with a focus on rapid assessment, diagnosis, and same-day management.

The post holder will work autonomously within their scope of practice, supported by the surgical multidisciplinary team, to assess and manage patients referred to SDEC. You will play a key role in facilitating admission avoidance, improving patient flow, and ensuring timely clinical decision-making in a fast-paced environment.

Consideration will be given to applicants in the last year of there MSc programme, As an Advanced Clinical Practitioner within the Surgical SDEC team, you will play a key role in delivering rapid, high-quality care to patients presenting with acute surgical conditions.

You will autonomously assess patients referred to SDEC, taking comprehensive histories, performing advanced clinical examinations, and formulating differential diagnoses. Using your clinical expertise, you will request and interpret investigations such as blood tests and imaging, and make timely decisions regarding patient management.

A core part of the role involves initiating treatment plans, including prescribing medications , and determining appropriate pathways whether that is same-day discharge, ongoing SDEC review, or escalation for surgical admission or intervention. You will perform clinical procedures within your scope of practice and ensure safe, effective, and evidence-based care throughout the patient journey.

Working closely with the wider multidisciplinary team, including surgeons, nursing staff, and allied health professionals, you will contribute to coordinated and efficient patient flow, supporting admission avoidance and optimising the use of hospital resources.

In addition to your clinical responsibilities, you will support the ongoing development of the SDEC service, participate in audit and quality improvement initiatives, and contribute to the supervision, teaching, and support of junior colleagues and learners.
