Summary
Greater Manchester Probation Service is strengthening its Health & Justice partnership offer to deliver whole-system change through effective collaboration and evidenced outcomes. The GM Health & Justice Partnership Co-Ordinator will play a key role in connecting probation with health and social care partners to reduce health inequalities, support safer sentence management, and improve continuity of care across the custody-to-community journey. The post will help drive delivery against our priorities: improving pre-release and resettlement planning for people with health needs including raising Continuity of Care performance, referral quality and access to pathways and improving performance and quality for Community Sentence Treatment Requirements and drug testing through stronger multi-agency coordination and assurance.
